The last Viceroy of India was LORD LOUIS MOUNTBATTEN.

One provision: two independent Dominions, INDIA and PAKISTAN, were to come into existence from 15 AUGUST 1947, each free to remain in or leave the Commonwealth. (Equally acceptable: British paramountcy over the Princely States lapsed and each was free to accede to either Dominion; the office of the Secretary of State for India was abolished; the Constituent Assemblies were free to frame their own constitutions.)

Explanation

One mark for the name and one for the provision. The date 15 August 1947 is the provision every examiner looks for first, so give that one unless the question narrows the topic. The chain is worth knowing: the Mountbatten Plan of 3 June 1947 was given legal effect by this Act, which received royal assent on 18 July 1947.
