Two things must both be right.
FIRST, the correlative pair: "not only" is always completed by "BUT ALSO", never by "as well as". That eliminates option (b).
SECOND, INVERSION. When a sentence BEGINS with "Not only", the subject and the verb change places: "Not only IS Dr. Shetty..." and not "Not only Dr. Shetty is...". This inversion is the rule candidates most often miss, and it is exactly why the instruction says "Begin with".
Option (a) inverts correctly but shifts the tense to the past ("was") when the original is present. Option (c) abandons the instruction and uses "both... and" instead.
