The two constructions take DIFFERENT words, and that is the whole question: prefer A TO B would rather A THAN B So "I prefer aerobics to yoga" becomes "I would rather do aerobics THAN yoga".
Why the others fail: (a) keeps "to" from the original construction — the commonest error, and exactly what the question is testing. (c) uses "prefer" and "rather" together, which is redundant; the instruction is to REPLACE the construction, not add to it. (d) reverses the preference entirely.
Also note the verb: "would rather" is followed by the BARE INFINITIVE — would rather DO, not would rather to do.
