The transformation from "too... to" into "so... that" follows a fixed pattern: too + adjective + to + verb so + adjective + THAT + subject + CANNOT + verb
Three elements must appear, and each wrong option drops one: (a) uses "so" twice instead of "so... that". (b) omits the "that" clause altogether. (d) drops the NEGATIVE, reversing the meaning — "too weak to walk" means he CANNOT walk.
Note the tense: the original is PRESENT ("is too weak"), so the modal is CANNOT, not "could not". The modal always follows the tense of the original sentence — a point the 2023 paper tests in exactly the same way.
