The two sentences combine with a NOUN CLAUSE introduced by "what": "I did not hear WHAT he said." The clause "what he said" acts as the object of "hear".
Option (d) is the trap and reverses the meaning entirely — the original says he did NOT hear. Always check the negative: in synthesis questions the distractors frequently flip it.
Option (b) is ungrammatical, because it uses both "something" and "what". The word "what" already means "the thing which", so adding "something" duplicates it. That double construction is a common error in Indian written English and is worth watching for.
