The shared element is a PLACE, so the relative adverb WHERE joins the two: "This is the hospital WHERE I was born."
Note that the word HERE disappears. It has been absorbed into "where", which already means "in that place". Writing "the hospital where I was born here" is redundant, and that redundancy is the commonest error in this question type.
The relatives, each tied to what it refers to: WHO — people; WHICH — animals and things; WHERE — places; WHEN — times; WHOSE — possession. "In which I was born" would also be correct but is heavier.
