The prostate gland lies JUST BELOW THE URINARY BLADDER, surrounding the URETHRA where it leaves the bladder.
The position explains the whole case in the question. Because the prostate encircles the urethra immediately below the bladder, surgery on it is carried out right next to the sphincter that holds urine in — which is why incontinence is a known side effect.
It also explains a common condition in older men: an enlarged prostate squeezes the urethra it surrounds and makes urination difficult.
Note that the prostate is part of the MALE REPRODUCTIVE system but sits directly on the urinary tract — a good illustration of why the two are together called the urinogenital system.
